VISA TO HAITI
Requirements for Visa to Haiti
1. Duly completed and signed Application form.
2. A passport valid for 6 months.
3. Two (2) passport-sized color photos;
4. Letter explaining the reason and length of stay, as well as providing references in Haiti;
5. Name, address, and telephone number of the hotel or hotels where the applicant will be staying; or alternatively, the name, address, and telephone number of the friend or friends with whom the applicant will be staying;
6. Photocopy of valid passport;
7. Photocopy of round-trip booking confirmation or ticket
8. Letter from the applicant’s current employer.
